                 The 1999 Tournament Entry Fee is $425
                          All checks must be in US Funds
   FIRST  POSITION  JERSEY #  All rostered players and 4 team officials
                            will receive admission passes.
                     Early admission passes for spectators can be
                   purchased for $4 each providing funds received
                    with this application. Admission at door is $6
                    Spectators 65 or older and 15 or younger will
                             be admitted at No Charge.
                  Teams scheduled to play between Noon and 6 pm
                  on Friday, February 5th, will receive tournament
                 entry fee reduction to $380 USD. We are looking for
                              8-10 teams to volunteer .
